Oral Surgery Referral Form

Thank you for selecting our office for your patient's oral surgery care. It is an honor that you have considered us. Rest assured that we will do our best to provide your patient with a pleasant dental experience each and every time that they visit our office.

A special note for patients that require emergency treatment:

We reserve time in our schedule daily for patients that require emergency care. Thus, we are usually able to see most emergencies the same day.

If possible, please send the patient with a referral form and any films that you might have. If no films are available, we will gladly obtain them here.
