What do I need to do before my treatment?

Please complete the following checklist before your visit.

  • Please fill out our patient registration forms before your appointment. For forms you can complete online, CLICK HERE.
  • Please alert the office if you have a medical condition that may be of concern prior to your visit (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), or if you are taking any medication that may be of concern(i.e. heart medications, aspirin, anticoagulant therapy, etc.).
  • Please call our office at least 24hrs prior to your visit and give us the names of ANY medications that you may be taking. By having this information in advance we will be able to research possible interactions between the medications you currently take and any that we may give you. Having this information prior to your visit will save you wasted time in the dental chair.
  • Please ensure that you have your current insurance information before your appointment, and bring your insurance card to the visit.
  • Patients that will be receiving IV Sedation and/or Nitrous Oxide Sedation please refrain from eating or drinking (not even water) for six (6) hours prior to your appointment.  In addition, you must have an escort that can drive you home.  They must be present for the entire procedure and cannot leave and come back.
  • Continue all medications for blood pressure, diabetes, thyroid problems and any other conditions as recommended by your physician. If there is a question, please call our office prior to your appointment.
  • If you have been advised by your physician or dentist to premedicate with an antibiotic because of a cardiac condition, a hip, a knee, a cardiac or other prosthesis, or if you have had rheumatic heart disease, please make sure you have taken the appropriate antibiotic on the day of your appointment.
  • Please eat a full breakfast or lunch as applicable.
  • If you are given X-rays or a Referral Note by your general dentist, please bring them to your appointment.
  • Please contact our office with any questions about payment requirements before your appointment.

IMPORTANT: All patients under the age of 18 years of age, and all patients who are still covered by a parent’s insurance plan, must be accompanied by a parent or guardian to the consultation visit.
