Our office will charge interest at 1.5% per month for all accounts over 60 days old.
If treatment is initiated but not completed, one half the full fee will be charged and collected at the time of the visit.
Quoted fees are guaranteed for one month.
Insurance companies tell patients what they consider "usual, customary and reasonable fees" (UCR) are for any given procedure. These fees are arbitrarily low and do not represent the true specialty fees in our area. Some specialists may charge a higher fee than our office and some lower for the same procedure. It is your responsibility to accept and consent to pay our fees before treatment. We will not adjust fees once treatment is completed.
